Below is a link to our parish census form. Please provide as much detail and accuracy as possible, for example grade level of your children and sacramental information. This form should include all adults and children living in your home that wish to become members of the parish. Once we have receive your completed census form, we will promptly enter your data into our parish census and register you as a parishioner.
